Applying for a Clipper Access Card

First-time applicants must apply in person for a Clipper Access card at Bay Area transit agency locations. Once an application is received, a verifier reviews it to ensure that it is completed and signed, with supporting documentation included. Qualified applicants are mailed a Clipper Access card typically within 21 days of approval.

How to Apply: Basic Eligibility

Visit the Clipper Access Eligibility page for a list of Basic Eligibility required documentation.

  1. Complete the Basic Eligibility Application (PDF).
  2. Present a valid government-issued photo ID (driver’s license, state ID card, passport, alien registration card, U.S. military identification card or military dependent card).
  3. Bring relevant required documentation listed on the application form.
  4. Your photo for the Clipper Access card will be taken at the transit agency where you submit your application.  

How to Apply: Medical Eligibility

Your Medical Eligibility Application must be certified by a California licensed medical professional. Visit the Clipper Access Eligibility page for a list of qualified professionals.

  1. Complete Section One of the Medical Eligibility Application (PDF).
  2. Sign Section Two and Section Three. Your signature in Section Two authorizes your doctor to release information; your signature in Section Three accepts the terms of the Clipper Access program. If your application is not signed, it will be considered incomplete and there may be delays in processing your application.  
  3. Bring the Medical Eligibility Application to your California licensed professional to complete Section Four to certify your disability and your need for an attendant, if applicable.
  4. Submit your completed application at your transit agency location. Agency staff will take your photo for the Clipper Access card.
